Critical size bone defects represent orthopedic challenges in fracture repair. Successful outcome often depends on the long-term integrity of the fixation method and the preservation, or restoration, of a biologically friendly environment. Using two clinical cases of failed osteosyntheses, this discussion will evaluate the causes of the original failure and describe the rationale for revision strategies.
